One of the most common conversations heard at family gatherings is a mother saying, “My child is not concentrating on studies,” or “My child is weak in mathematics.” Most people assume that poor academic performance is caused by a child’s behavior, lack of interest, or laziness. However, scientific research tells a different story.
Studies have consistently shown that iron deficiency and other micronutrient deficiencies significantly reduce brain function. Iron plays a vital role in oxygen transport and cognitive development. A deficiency can impair concentration, memory, learning ability, and overall mental performance. Unfortunately, these symptoms are often misunderstood as behavioral problems rather than nutritional deficiencies.
This condition is known as Hidden Hunger—a deficiency of essential vitamins and minerals despite consuming enough calories. Many people believe that eating three meals a day is sufficient for good health, but a full stomach does not always mean a well-nourished body. Modern food systems, increasing commercialization of the food industry, changing dietary habits, and declining nutrient density in crops have contributed to widespread deficiencies of iron, zinc, folic acid, vitamin B12, and other essential micronutrients.
Pakistan is currently facing a serious nutrition emergency. National surveys continue to highlight alarming levels of malnutrition, stunting, anemia, and micronutrient deficiencies among women and children. According to the National Nutrition Survey (NNS) 2011, 51% of pregnant women were anemic, 37% were iron deficient, 46% suffered from vitamin A deficiency, and 69% were deficient in vitamin D. Among children under five years of age, 62% suffered from anemia, while 54% were deficient in vitamin A.
These deficiencies weaken the immune system, increase susceptibility to disease, contribute to maternal and child mortality, and negatively affect children’s cognitive development and educational outcomes. Poor nutrition during early childhood ultimately leads to lower productivity in adulthood, placing a significant burden on the national economy. Pakistan is estimated to lose approximately 2–3% of its GDP annually due to the economic consequences of malnutrition.
One of the most effective, evidence-based, and cost-efficient interventions to address hidden hunger is food fortification—the addition of essential vitamins and minerals to commonly consumed staple foods.
During my tenure as Zonal Manager, Large Scale Food Fortification Program with Nutrition International, I had the opportunity to contribute to one of Pakistan’s most impactful public health initiatives. The program provided both financial and technical assistance to the wheat flour milling industry by installing micro-feeders capable of accurately adding internationally standardized vitamin and mineral premixes into wheat flour.
The premix contains iron, zinc, folic acid, and vitamin B12, which are added at approximately 200 grams per metric ton of flour. This innovative approach enables millions of consumers to receive essential micronutrients every day without changing their eating habits. Since wheat flour is consumed daily by almost every household in Pakistan, fortification provides an efficient platform for improving maternal and child nutrition at the population level.
The Government of Sindh, through the Health Department and in partnership with the Food Fortification Program (FFP), has taken significant steps to strengthen public health through mandatory wheat flour fortification. Roller flour mills are required to fortify Atta, Fine Flour, and Maida with iron, folic acid, zinc, and vitamin B12 according to prescribed standards. Initially implemented in Karachi, the program continues to expand across Sindh Province with the objective of reaching every community.
More recently, while serving as Manager, Resource Development at the Fortify Education Foundation, I prepared a grant proposal for the Commonwealth Health Justice Program aimed at addressing adolescent and child malnutrition through nutritious school meals. The proposed intervention promotes daily consumption of chickpeas, eggs, chicken, and fortified wheat flour while simultaneously improving school attendance, educational outcomes, and long-term health.
The challenge of micronutrient deficiencies is not unique to Pakistan. Across the Middle East and North Africa, vitamin D deficiency remains widespread despite abundant sunshine, primarily because of limited effective sun exposure and dietary inadequacies. This highlights that hidden hunger is a global public health issue requiring integrated nutrition policies, food fortification, supplementation where necessary, nutrition education, and stronger food systems.
Food fortification alone cannot eliminate malnutrition, but it remains one of the most affordable and sustainable public health interventions available. Its success depends on mandatory legislation, effective regulatory compliance, rigorous quality assurance, industry participation, and continuous monitoring by government institutions.
The true impact of these interventions will become more evident with future National Nutrition Survey findings. Until then, continued collaboration between government, development partners, academia, the private sector, and civil society remains essential to improve the nutritional status of the population.
Pakistan’s future depends on healthy mothers and healthy children. By investing in food fortification today, we are investing in stronger minds, healthier communities, greater economic productivity, and a brighter future for generations to come. Hidden hunger may be invisible, but its consequences are not. Together, we can ensure that every meal nourishes not only the body but also the nation’s future.
